St. Matthias CYO 2010 Baseball, Softball and Tee-Ball Registration

• Fill out all parts of the form. The form will not process if any required sections are blank.
• You will receive email confirmation that your form was processed. Please let us know if any information on your email confirmation is incorrect.
• Fees are due by March 15, 2010. Participation fees will be $90 for 3rd through 8th graders and $45 for K - 2nd grade participants. Maximum fees per household will be $165.
